To fill out the affiliation form, please note the following:

- Most fields on the form are mandatory (indicated by an *).
- You will need the names, email addresses, and mobile numbers of all the teachers you want to affiliate.
- We allow up to 5 teachers to be affiliated per studio. (See “How many teachers are you affiliating?” on the affiliation form.)
- As of 2025, affiliation is still mandatory, but free. Instead, student registration carries a fee of R30 per student.
- Clicking “Submit Affiliation Form” on the form, will send an email to your studio’s email address (as entered into the form) with a link to verify your EPA Dance account.
- Note: You will need to log in using your verified email address as your username to access Studio & Teacher content on our website.
- When your account is successfully verified, you can access additional information about the 2026 Eisteddfod such as Guidelines and Set Solo music & videos by using the “Teacher Login” menu option.
